Urgent Appeal: Help Fund 4-Year-Old Necla’s Life-Saving Liver Transplant

Her condition is worsening, she urgently needs your support.

Urgent Appeal: Help Fund 4-Year-Old Necla’s Life-Saving Liver Transplant

Four-year-old Necla Razqya Fema has been battling severe liver disease since May 2024. Over time, her condition has deteriorated, leading to life-threatening complications, including: 

  • hepatic vein thrombosis, 
  • cirrhosis, and 
  • severe malnutrition. 

She now requires an urgent liver transplant to survive.

Her parents, desperate to save her, have already sold everything to afford medical care and traveled from Batam, Indonesia, to Malaysia in search of specialists. 

However, the cost of a life-saving transplant is beyond their means. They need our help to ensure Necla gets the chance to live.

Understanding Necla’s Condition

Cryptogenic Chronic liver disease Ddx Schistosomiasis

Necla has been diagnosed with chronic liver disease. Her liver has been slowly deteriorating over time.

However, doctors have been unable to determine the exact cause, making her condition cryptogenic (of unknown origin).

  • Ddx stands for “differential diagnosis”, a process doctors use to determine a patient’s illness or condition when the symptoms are unclear.  

One possible explanation being investigated is schistosomiasis, a parasitic infection that can cause liver damage.

While this is still under evaluation, the lack of a clear cause makes her treatment more challenging.

Cirrhosis

Unfortunately, her liver disease has already progressed to cirrhosis. Cirrhosis is a condition where the liver becomes scarred and permanently damaged.

This means scar tissue has replaced healthy liver cells, leading to serious complications such as,

  • fluid buildup,
  • bleeding, and,
  • difficulty processing nutrients.

Hepatic Vein Thrombosis & Budd-Chiari Syndrome

Hepatic vein thrombosis is when blood clots have blocked the veins that drain blood from the liver.

This condition has led doctors to diagnose Necla with Budd-Chiari Syndrome, a rare condition where the veins that carry blood from the liver get blocked.

As the blood is unable to flow out of her liver properly, it is causing severe damage to her liver and other organs. This blockage has led to:

  • Liver and spleen swelling (Hepatosplenomegaly) – Her liver and spleen have become abnormally enlarged due to the blood flow being obstructed.
  • High blood pressure in the liver’s veins (Portal Hypertension) –This happens because the blockage is stopping blood from flowing normally, which increases pressure in the liver. This has caused the veins in her esophagus to swell, and they might burst, which could be very dangerous.

  • Fluid buildup in the belly (Ascites) – Since her liver is not working properly, fluid is leaking into her abdomen, making her stomach look swollen and causing discomfort.

If untreated, Budd-Chiari Syndrome can lead to complete liver failure—which is why Necla needs a transplant urgently.

Severe Malnutrition

At just 11.2 kg and 93 cm tall, Necla is significantly underweight and malnourished for her age. 

This is because her diseased liver cannot properly absorb nutrients, leading to muscle loss, weakness, and stunted growth.

Her mid-upper arm circumference (MUAC) is only 10 cm, a clear sign of severe malnutrition. 

Her Family’s Struggle to Save Their Daughter

Necla’s parents have exhausted all their financial resources trying to find the best care for their daughter.

After receiving conflicting diagnoses in Indonesia, they traveled to Malaysia, where detailed scans confirmed that only a liver transplant could save her life.

They are desperate to give their daughter a chance to grow up healthy—but they cannot afford the cost of the transplant alone.
